Disappearing PRs

Hi all,

We are having issues with some pull requests in a Private repo, when they are opened, for some reason they are disappearing and cannot be accessed (404).

When we attempt to open a new PR we get: Pull request creation failed. Validation failed: A pull request already exists for :.

Does anybody have any ideas? This is a free account right now.
